Scheduling Appointments

We currently have four locations in central Indiana:

  • Danville, just west of Indianapolis;

  • Plainfield

  • Downtown Indianapolis, near Mass Ave.

  • Lafayette

  • We also offer virtual Telehealth appointments with all providers for most appointment types (please contact the office directly for details)

Routine office hours are Monday through Friday from 8:00 am – 5:00 pm, though many providers see patients after hours and on weekends. All services are by appointment. Our staff will work with you to find the best available appointment time for you.

Current Patients - To schedule an appointment, contact us and we will be glad to assist you with questions and scheduling.

APPOINTMENT CANCELLATION

If an appointment needs to be cancelled, we appreciate as much notice as possible and request a minimum notice of 24 hours. This courtesy on your part allows us time to schedule another patient in need. In most circumstances, you may switch to a Telehealth appointment if you cannot make it into the office for your scheduled appointment time. Cancellation fees will be applied for missed appointments. MISSED APPOINTMENTS POLICY

Your appointment is reserved just for you, and we look forward to meeting you at your reserved time. If you miss an appointment without sufficient notice, this means that another person is not able to use that appointment time. We currently have hundreds on our Waiting list anxious to schedule an appointment with a provider, lack of notice denies another an opportunity to get the critical care they may seek.


Unless there are circumstances your provider and you would define as an emergency, it is our policy to charge a fee of up to $90 when your appointment is missed or not cancelled 24 hours in advance. If there was a sudden emergency or unavoidable scheduling conflict, please see your provider about rescinding the charges.

Repeated late cancellations or missed appointments may make it impossible for us to continue serving you as a patient, and may require us to release you from our care.
