To schedule an appointment, you will need to complete the registration form. Since you are a returning patient, you will not need to complete the “Personal and Medical/Psychiatric History” section unless there have been changes. Please list any NEW changes to your health information and medications on the registration form since your last visit. Call our office if you have any questions.

Will My Appointments Be Covered By Insurance?
We take many insurance plans. We have listed some insurance carriers on the Billings and Insurance page. Additional ones may not be listed, so we advise you to check with your insurance company concerning your mental health benefits.
What if I Don’t Have Insurance and Want to Self-Pay?
Self-pay is available. We understand that seeking treatment for your mental health can be challenging. Some patients with and without insurance prefer to self-pay for their sessions.
What if I Don’t Like My Therapist?
We will schedule you with another therapist at their earliest opening so that you are comfortable at your next session.
